Cat Adoption Application

Cat Adoption Application and Questionnaire Form

"*" indicates required fields

Thank you for your interest in adopting a cat! We strive to make the best possible match between our rescued cats and their new families. The Adoption Application form is the first step to see if the cat you are interested in would be a good match for your family. Once submitted, one of our volunteers will review your information and will be contacting you by email to discuss the information you provided. Please be sure to check your Spam folder in case our email reply goes there. MINIMUM REQUIREMENTS TO ADOPT:
  • Be able to provide us with a photo ID or other paperwork verifying your current address
  • Be at least 21 years old
  • Have confirmation from your landlord that pets are allowed, or own your own home
  • Be able and willing to spend the time and money necessary to provide training, medical treatment and proper care for your cat.
Please fill out the form completely - we will discard any application that is not completely filled out. If a question doesn't apply to you please enter N/A or explain otherwise. Submitting an application does not obligate you to adopt nor does it guarantee the pet will be adopted to you. We collect applications on the pet in question and make our best determination on where we think the animal will do best.
Enter N/A if this question doesn't apply

Making A Commitment

Adopting a cat is a lifelong commitment. Are you aware that a cat can live to be over 15 years old? Are you prepared to make this commitment to this animal?
Are you aware that cats can chew, scratch, nibble, and in some cases bite? That they can make noises, such as meowing, growling, whining, hissing, etc? Are you prepared for this?

A.A.R.F. makes every attempt possible to find forever homes for these animals. Relocating animals from home to home has a negative affect on animals, therefore, we strive to do it right the first time. Rescue animals in general come with some type of emotional baggage. Since we were not there to see what they have experienced and witnessed, we cannot guarantee you the perfect animal immediately. All animals take patience, time, work and commitment - some rescue animals may exceed the normal amount that one would expect.

By selecting the "I agree" option, you are stating that you understand the mission of this organization; you understand what is expected of you in regard to caring for this animal; and you are ready, willing, and able to make this lifelong commitment to this animal.
